In December of 2014, Scott Greenfield arrived at work to perform his regular job duties. But at around 8:30am, the 42-year old father of four suffered a crushing blow to his head at his place of work at a plastics plant. He later died from his head injuries.
There is no question that the situation was unfortunate, but the question which plagues his wife, Pattie Greenfield, is whether or not the accident was preventable. While the death of Greenfield was ruled accidental by the coroner, it was found that the plant had 17 violations as per the state’s Occupational Safety and Health Administration’s (OHSA) April inspection. 5 of those violations were deemed to be “serious”.

Greenfield’s wife bitterly recalls the incident, stating that she remembered holding his hand and then suddenly she would never be able to hold his hand again.
